Survey dates that are less than two weeks of the order date require expedited shipping costs of $100 and may not be possible to honor. Research Services staff will work with you to confirm your survey date.

Upon submitting your order form, you will receive an email from someone in our office with a project blueprint, which you will need to sign and return. Please allow up to one week for this email. After signing the blueprint, we will provide you with everything you need to field the survey. Invoices are sent after the blueprint is signed.
